Medical Patients:

Neither ADD ExPRESS Transportation nor the driver for ADD ExPRESS Transportation are responsible for aiding any patients needing medical assistance. If a patient need an aid to assist him/her, they will be required to have a signed medical form from their medical adviser showing the location for their facility. The aide will be picked up and dropped off at the same address as the client.

A companion is anyone traveling with the client. Companions may travel with the client, but will be charged the same as a regular client. "Companions" does not mean the same as an aide. An aide must have a signed certification form permitting him/her to assist the client. Companions must be picked up and dropped off at the same address as the client if they’re gonna be traveling with the client. They must inform dispatch when they will be traveling with the client.

Failure to properly cancel a scheduled trip or non-scheduled trip causes an inconvenience to other riders.

(1) No Show————————–Customer is not at the scheduled pick-up location at the established ready time.
(2) Late Cancellation—————-Customer cancels a scheduled trip when driver arrives at their door at established ready time.and chooses not to use ADD ExPRESS
Transportation.

For members or non-members, all charges must be paid before other services can be rendered. Riding privileges are revoked until all late/cancellation charges have been paid.
